JOB SUMMARY
The MDS Coordinator is responsible for coordinating and completing the resident assessment process in accordance with federal and state regulations for long-term care facilities. This role ensures accurate and timely completion of the Minimum Data Set (MDS), care planning, reimbursement optimization, and interdisciplinary communication to support quality resident care. Utilizes the nursing process to assess, prioritize, plan, deliver, and evaluate adolescent, the adult and geriatric resident population responses.
Qualifications:JOB QUALIFICATIONS
· Current Registered Nurse (RN) license in the applicable state.
· Experience in long-term care or skilled nursing preferred.
· Knowledge of MDS 3.0, RAI process, PDPM, Medicare, and Medicaid reimbursement systems.
· Familiarity with CMS regulations and documentation standards.
· Strong organizational, analytical, and communication skills.
· Experience with electronic medical record (EMR) systems preferred.
· RAC-CT certification preferred but not always required
