Case Manager - TLS ProgramAs a Case Manager in the TLS program, you'll help stabilize housing, connect participants to vital resources, and empower them toward independence.Responsibilities:Manage a caseload of up to 20 participants, conducting monthly home visits and providing ongoing support.Assist clients with employment, benefits, relocation, substance use, and mental health barriers.Maintain accurate documentation in HMIS and complete required administrative tasks.Collaborate with program managers and team members to ensure high-quality service delivery.Requirements:A minimum of 1 year of recent experience in case management working with vulnerable populations.Strong cultural competency and ability to work with diverse backgrounds (LGBTQ, undocumented, etc.).Excellent time management and organizational skills.Reliable transportation and a valid driver's license required.Willingness to travel throughout LA County for field work main focus in SPA 7; mileage is reimbursed.Ability to work Monday-Friday, 8:30am-5:00pm.Comfortable with a temporary assignment (90 days to start, possible extension).Pay Details: $23.00 to $26.00 per hourSearch managed by: Joanna Calderon Benefit offerings available for our associates include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and a 401K plan.
