Job Overview
We are seeking a compassionate and dedicated Case Manager to join our team in providing comprehensive support to individuals and families facing various challenges. The ideal candidate will have experience in working with homeless, those in addiction, and/or crisis situations. This role involves coordinating services, advocating for clients, and ensuring they access necessary resources to improve their well-being. Duties
- Conduct thorough assessments of clients’ needs, including behavioral health, addiction counseling, child welfare, and workforce development.
- Develop individualized care plans that address clients’ goals and connect them with appropriate services such as Section 8 housing, DMV, counseling, employment, education, recovery, and other housing programs.
- Provide crisis intervention to stabilize clients during emergencies or high-stress situations.
- Utilize motivational interviewing techniques to motivate clients toward positive behavioral changes and recovery.
- Monitor progress regularly and adjust care plans accordingly to ensure optimal outcomes.
- Collaborate with healthcare providers, social service agencies, and community organizations to coordinate comprehensive support services.
- Maintain accurate documentation of client interactions, service plans, and progress notes in compliance with agency policies.
Experience
- Demonstrated crisis intervention and ability to bring calm in high-pressure environments.
- Knowledge of addiction counseling techniques and behavioral health best practices.
- Familiarity with programs such as Section 8 housing, DHHR, recovery, and other social service resources is advantageous.
- Strong communication skills with the ability to engage clients empathetically while maintaining professional boundaries.
- Experience working with vulnerable populations including children, families, or individuals facing mental health challenges is highly desirable. This role is vital in supporting individuals on their journey toward stability and independence through dedicated case management services delivered with professionalism and compassion.
Pay: From $12.00 per hour
Expected hours: 24.0 per week
