The Case Manager is required to possess a minimum of a Bachelor’s Degree in human services or related field such as education, sociology, etc. and preferred two years experience working with targeted population..
Must be over 21 years of age with a valid driver's license.
Advocates for clients, provides services and assists clients to gain access to services as determined by the client’s individualized treatment plan.
Provides individual and group supportive counseling.
Completes and maintains required accurate documentation, in a timely manner.
Participates in or directs meetings such as individual treatment planning, administrative review, treatment team and discharge planning.
Completes client’s intake.
Assists in development and implementation of Individual Treatment Plans and Behavior Management Plans/Protocols as necessary.
Attends all meetings relevant to assigned clients, both on and off campus.
Serves as a liaison between EMS and clients, service providers, family members, court officials, probation officers, lawyers and other agencies.
Assumes Case Manager on-call duties as scheduled.
Consults with psychiatrist/psychologist.
Consults with physician.

Pay: $18.05 - $28.76 per hour
