Overview:
We are looking for a committed Physical Therapist to join our Lamoille Home Health & Hospice team full- time. These services are home based, professional judgement and a strong knowledge of autonomy is highly valued. The ideal candidate will have a passion for patient care, ability to plan, implement, and coordinate a physical therapy program from eligibility determination to exit. We are offering a limited time sign on bonus of $5,000 if hired full- time (29 hours)(paid out over 6 months).
Duties:
- Conduct patient assessments and develop individualized treatment plans
- Implement therapeutic exercises and modalities to aid in patient recovery
- Educate patients and their families on treatment plans and home exercises
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Maintain accurate patient records and documentation
Requirements:
- Degree in Physical Therapy from an accredited program
- State licensure as a Physical Therapist
- Strong knowledge of medical terminology, anatomy, and physiology
- Experience in home health preferred but not required
