Job description:
We are seeking a dedicated and compassionate Intake Specialist with a background in nursing, licensed practical nursing (LPN), or medical assisting (MA) to join our dynamic healthcare team. This vital role involves coordinating patient intake processes, assessing patient needs, and facilitating seamless communication between patients and healthcare providers, coordinating medical services, and ensuring the highest quality of healthcare delivery.

Job Summary:
· Collaborate with facilities, physicians, patients and families to optimize patient care
· Triage patient calls
· Coordination of patient visits
· Chart and data entry
· Manage incoming referrals and faxes
Requirements:
· Candidates should possess substantial clinical experience or proven experience in home health, hospice and community health setting
· Familiarity with HIPAA regulations and medical ethics
· Knowledge and ability to navigate electronic medical records software
· Strong organizational and communication skills
· Ability to work well under pressure and handle multiple tasks efficiently
Pay: $26.03 - $35.00 per hour
