As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. You will partner with the Clinical Director and Executive Director to establish and maintain a therapeutic environment and routine for our residents.

Essential Functions:
·Work with the Clinical Director to develop, implement, and maintain a therapeutic environment and routine for residents.
·Assess the needs of residents according to DSM 5 criteria to provide proper treatment planning.
·Assess the effects of services on other community members including family members, friends, and social agencies. Engage in discussions with these agencies to determine needs and participate in planning referrals.
·Provide case management services for assigned caseload of patients. Provide treatment planning services as needed.
·Maintain contact with community resources including medical care providers, social services agencies, court systems, etc., to facilitate community reintegration into community resources.
·Participate in community resource coordination meetings with appropriate agencies and court systems.
·Consult in accordance with community resources as needed.
·Perform other duties as assigned.
Required Skills:
- High School diploma or equivalent required (Bachelor’s Degree preferred)
- Experience working with individuals suffering from a mental or drug diagnosis strongly preferred. (Preferred but not required)
- COVID –19 Vaccination approved exemption status preferred
Job Type: Full-time
Pay: $38,949.85 - $45,498.74 per year
