What are the responsibilities and job description for the Case Manager position at A1 Case Management?
Want to make a difference in the lives of people? This case management position utilizes Person-centered practices to assess, facilitate, and monitor the development and implementation of a plan of care for Participants in the Kentucky Medicaid Waiver programs. Our case managers work as a "team" with other agencies providing services to participants.
Applicants should have a bachelor's degree in a human service-related field but allowances can be made for experience in the field counting toward degree requirements. Must have at least one year of direct contact with individuals with intellectual or developmental disabilities. No case management experience is required and all training is provided.
