What you’ll do
- Assess patients in their homes and develop individualized care plans
- Provide skilled nursing care, including wound care, medication management, disease management, and post-hospital recovery support
- Educate patients and families so they feel confident managing their health between visits
- Coordinate with physicians, therapists, and the rest of our care team
- Monitor progress, spot changes early, and advocate for your patients
- Keep accurate, timely documentation and help drive quality improvement
- Join team meetings, quality improvement work, and continuing education

What we’re looking for
- Active RN license in Wyoming or a compact state, or eligibility to obtain one
- Strong clinical assessment skills and sound independent judgment
- Warm, clear communicator who connects easily with patients, families, and teammates
- Able to safely handle the physical demands of patient care in the home
- Valid driver’s license, reliable transportation, and current BLS
- Home health experience is a plus, but med-surg, ICU, ER, and other acute care backgrounds translate well. Newer nurses with solid clinical skills are warmly encouraged to apply.

Pay: $32.00 - $42.00 per hour
