- Reviews and analyzes medical records after discharge for completeness in compliance with the hospital policies and procedures, federal and state requirements.
- Collects all discharged patient charts from the patient care units daily.
- Assists in coordinating the release of health information to all requestors, including patients, physicians, and other healthcare providers.
- Collects loose filing for those discharged charts and inserts such filing in proper chart.
- Combines the current patient chart with any previous patient discharges for a unit record.
- Assembles discharged patient charts according to assembly list.
- Places assembled chart in proper folder according to medical record number.
- Analyzes medical records of discharged patients for electronic and hard copy for completeness and accuracy according to department and hospital policy and procedures.
- Pulling/retrieving/sorting, document assessment, prepping, scanning/indexing, and quality analysis of the original paper record documents into any record scanning application/electronic medical record system.
- Assists in tracking/monitoring physician deficiencies in the EHR and on deficiency shelf.
- Forwards all analyzed charts to the Coder.
- Demonstrates knowledge of computer systems.
- ssist in completing departmental/hospital quality improvement reviews/audits.
- Assists in maintaining security and confidentiality regulations for healthcare information access and review. Consults supervisor of any unusual request.
- Assists in performing SNC reviews/audits using established criteria and completing summary reports. Is familiar with SNF charting requirements. Assists with MDS functions/review.
- Assists in transcribing reports and other dictation as requested.
- Assists coder by preparing ED, OP charts and abstracting charges.
- Assists in processing appropriate paperwork for completing/obtaining birth/death certificates.
- Coordinates the release of health information to all requestors, including patients, physicians, and other healthcare providers.
- High school graduate or equivalent required with minimum one year or more of previous similar hospital experience preferred.
- Knowledge of medical terminology and California state medical charting requirements.
- Must be able to read, write and understand English and interface effectively and tactfully with patients, staff, and the public.
- Basic computer skills with the ability to download and upload documents required.
- Must maintain Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security regulations for protected health information, access, confidentiality, and review.
