Kahu Malama Nurses is seeking a Travel Physical Therapist to join a growing and dynamic team!
Job Description:
- Provide home health physical therapy services to patients to restore function, mobility, and independence.
- Assess and evaluate patient needs and develop individualized treatment plans.
- Document patient progress, assessments, and discharge notes accurately in OASIS.
- Educate patients and caregivers on exercise programs, safety techniques, and use of assistive devices.
- Collaborate with interdisciplinary teams to ensure coordinated and comprehensive care.
- Travel between patient homes and float to cover Oahu, Maui, and/or the Big Island as needed (expenses paid).
- Maintain compliance with all safety, documentation, and quality standards.
- Current Hawaii Physical Therapy License and BLS (AHA) certification required.
- At least 1 year of experience in a home health environment.
- Strong knowledge of geriatrics, orthopedics, neurology, and post-stroke rehabilitation.
- Proficiency in patient assessment, treatment planning, and mobility training.
- Ability to lift and move patients safely using proper body mechanics.
- Excellent communication, documentation, and time management skills.
- Commitment to safety, patient education, and continuous quality improvement.
