Job Description
- Conduct assessments to identify residents' psychosocial needs.
- Develop individualized care plans in collaboration with residents, families, and healthcare providers.
- Provide counseling and support to residents and families to address emotional and social issues.
- Advocate for residents' rights and coordinate with external agencies as needed.
- Maintain accurate and timely documentation of services provided.
- Participate in care conferences and multidisciplinary team meetings.
- Collaborate with healthcare professionals to ensure holistic care.
- Investigate grievances and alleged abuses.
- Assist with admission and discharge planning.
- Proficiency with database software, Microsoft Office products, and office equipment.
- Strong communication and interpersonal skills.
- Ability to work collaboratively with a multidisciplinary team.
- Knowledge of social service resources and agencies.
