Location: Honolulu, HI

Pay: $84,00 to $85,000/year

Experience: Previous experience working in a nursing home or long-term care setting.

Education: Bachelor’s or Master’s degree in Social Work or a healthcare-related field.

Type: Full-time; Direct Hire

Schedule: Monday – Friday, 8:00 AM to 4:30 PM

Kahu Malama Nurses is seeking a Social Worker to join a growing and dynamic team!

Job Description

• Conduct assessments to identify residents' psychosocial needs.
• Develop individualized care plans in collaboration with residents, families, and healthcare providers.
• Provide counseling and support to residents and families to address emotional and social issues.
• Advocate for residents' rights and coordinate with external agencies as needed.
• Maintain accurate and timely documentation of services provided.
• Participate in care conferences and multidisciplinary team meetings.
• Collaborate with healthcare professionals to ensure holistic care.
• Investigate grievances and alleged abuses.
• Assist with admission and discharge planning.
  
  

Position Requirements

• Proficiency with database software, Microsoft Office products, and office equipment.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with a multidisciplinary team.
• Knowledge of social service resources and agencies.
