- 1 year of Home Health experience is required.
- 3 months of OASIS documentation experience is required.

Kahu Malama Nurses is seeking a Registered Nurse (Home Health) to join a growing and dynamic team!
Position Requirements
- Experience providing skilled nursing care in an adult home health setting, including admissions, Start of Care, recertifications, resumptions of care, routine visits, transfers, and discharges.
- Experience conducting comprehensive patient assessments, including significant changes in condition, nutritional needs, pain and medication management.
- Strong case management skills with the ability to coordinate patient care and services.
- Experience with wound care, pressure ulcer prevention and staging, wound V.A.C. management and post-surgical care.
- Ability to provide care for patients with feeding tubes, G-tubes, NG tubes, colostomies, ileostomies, urinary catheters, suprapubic catheters, drains and AV fistulas/shunts.
- Experience managing peripheral IVs, central lines and ports, including IV starts, phlebotomy and administration of blood or blood products.
- Knowledge of blood glucose monitoring, oxygen administration and safety, infection control precautions and obtaining vital signs.
- Experience caring for patients with casts, braces, immobilizers, amputations and total joint replacements.
- Ability to monitor chemotherapy side effects and provide pain management, including hospice-related care.
- Strong clinical assessment, documentation and care coordination skills.
- Experience providing psychiatric home health services.
- Tracheostomy care experience.
- Experience with HCHB documentation.
