Job Summary:
JCCA is seeking a qualified Licensed Practical Nurses to provide primary health care management for assigned cases collaborating with all members of health care and clinical team.
Summary of Job Functions:
- Provide primary health care management for assigned cases.
- Maintain accurate and up-to-date Medication Administration Records
- Actively communicate with community providers scheduling needed appointments and obtaining health related information for clients
- Communicate and coordinate with clinical team, foster and biological family/guardian about all medical conditions, appointments and/or treatments.
- Provide formal and informal training for foster and biological parents on specific health care needs of youth, including appropriate health practices and administration of medications.
Qualifications:
- NYS Licensed Practice Nurse required
- Pediatric and/or Medical/Surgical experience
- Excellent organizational skills and follow up.
- Effective communication skills - both written and oral
