Case Planner I (FHS - EFFC)

Position Summary:
JCCA is seeking passionate and dedicated Case Planners to help ensure every child in foster care achieves a safe and permanent home through reunification, adoption, or preparation for independent living. As a Case Planner, you’ll be trained and supported in implementing evidence-based models that empower families and caregivers to create lasting change. This is a meaningful opportunity to make a direct impact on children and families across New York City.

Essential Responsibilities:
• Partner with families to develop plans addressing safety, behavioral, psychological, and permanency needs.
• Monitor safety, well-being, and permanency progress through regular casework contacts and tailored services.
• Complete timely and accurate documentation, including FASPs, psychosocial assessments, adoption home studies, kinship home studies, CANS assessments, and court reports.
• Actively participate in Family Court Hearings, Family Team Conferences, Administrative meetings, and other forums to advance permanency goals.
• Provide crisis intervention and implement evidence-based models with foster parents, parents, discharge resources, and youth.
• Coordinate with internal programs (e.g., Behavioral Health & Wellness, Health Homes) and external community resources.
• Provide family planning counseling to youth 12 and document discussions with foster parents.
• Participate in all required trainings to strengthen practice and model implementation.
• Utilize Family Finding evidence-based practices to identify supportive family resources for youth.

Knowledge, Education, and Skills Required:
• Bachelor’s degree in Social Work, Human Services, Psychology, or related field required; Master’s preferred.
• Experience working with families and adolescents in the child welfare system.
• Strong ability to build relationships with youth and families in need of support.
• Comfort working with diverse populations and interdisciplinary teams.
• Excellent organizational, verbal, and written communication skills.
• Ability to work both independently and collaboratively.
• Flexible schedule, including some evening hours.
• Ability to travel throughout NYC via public transportation or agency vehicle.
• Commitment to strength-based, youth development, and trauma-informed approaches.
• Proficiency in MS Office and ability to learn new systems.
• Valid NYS Driver’s License preferred.

Hourly Rate: $27.47 – $29.71 (depending on experience) differential offered for Spanish or Creole language skills
