Rates for PT
- New patient SOC: $115
- Initial evaluation: $100
- Recertification/ROC: $100
- Add on recertification: $90
- 30-day reassessment: $95
- Regular visit (PT) $85
- Discipline discharge: $90
- Agency discharge: $95

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:
- Physical Therapist will Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
- Physical Therapist will Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
- Physical Therapist will Observe movements, conduct assessments, document progress and modify care plan as needed
- Assist patient with exercises and hands-on therapy to manage pain and increase mobility
- Educate patients on their plan of care, established goals, exercises and self-care
- Coach family on the plan of care, therapy goals and safely assisting with exercises
A few must-haves for Home Health Physical Therapists:
- Education: Graduate of an accredited Physical Therapist Program.
- Licensure: Active Physical Therapist (PT) License in the state of Maryland.
- Experience: Minimum of [2] years of physical therapy experience (Home Health experience preferred).
- Certifications: Current BLS Certification.
- Health Compliance: COVID-19 and Flu vaccinations required.
- Skills: Strong interpersonal communication, dexterity, and knowledge of federal/state home health regulations.
