📍 Location: Oxford, New York
🕒 Schedule: Full-time | Monday to Friday | 8:00 AM to 4:30 PM (40 hours/week), Shared on-call coverage

What You'll Do:
- Conduct routine and as-needed patient evaluations.
- Diagnose, treat, and manage acute and chronic medical conditions.
- Develop, implement, and monitor individualized care plans.
- Collaborate with physicians, nursing staff, rehabilitation teams, and interdisciplinary care providers.
- Maintain accurate and timely documentation within the electronic medical record (EMR) system.
- Participate in care conferences and quality improvement initiatives.
- Provide clinical support for skilled nursing and rehabilitation services.
- Share responsibility for 24/7 on-call coverage as part of the provider team.
Qualifications:
- Active New York Nurse Practitioner or Physician Assistant license.
- Current DEA registration.
- BLS certification.
- Experience in skilled nursing, long-term care, geriatrics, rehabilitation, internal medicine, or family medicine preferred.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
