Job Summary: The Nurse Case Manager II (RN) is responsible for delivering comprehensive case management services through a collaborative process of assessment, planning, facilitation, and advocacy. This role focuses on coordinating care, evaluating member needs, and ensuring optimal, cost-effective health outcomes through effective communication and resource utilization.
Key Responsibilities:
- Conduct comprehensive clinical assessments using data, clinical tools, and member history
- Evaluate member eligibility, benefits, and healthcare needs to determine appropriate care plans
- Develop and implement case management strategies aligned with clinical guidelines and policies
- Coordinate care across internal teams and external healthcare providers
- Advocate for members to ensure access to appropriate services and resources
- Apply regulatory standards, policies, and procedures in all case management activities
- Monitor and adjust care plans to ensure quality and cost-effective outcomes
- Maintain accurate documentation and reporting in multiple systems
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in New Jersey (Compact license accepted)
- Minimum 3 years of clinical experience (hospital, home health, or ambulatory care)
- Strong computer proficiency (ability to navigate multiple systems)
- Excellent verbal and written communication skills
- Ability to multitask and adapt in a fast-paced environment
Preferred Qualifications
- Case Management experience
- CCM (Certified Case Manager) certification
- Experience in managed care or healthcare industry
- Candidates located in North Jersey counties (Sussex, Warren, Bergen, Essex, Passaic, Hudson)
