Clinical Care Nurse RN

The Clinical Care Nurse (RN) is a remote, clinic-based role focused on improving patient outcomes through quality improvement, Transitions of Care (TOC), and population health initiatives. This position plays a key role in reducing avoidable hospital utilization, supporting post-discharge care, and driving Medicare Advantage Stars and HEDIS performance. The RN will combine patient outreach and education with data-driven strategies to identify care gaps, engage patients and providers, and implement evidence-based interventions to improve overall health outcomes.

The Clinical Care Nurse (RN) is a remote, clinic-based role focused on improving patient outcomes through quality improvement, Transitions of Care (TOC), and population health initiatives. This position plays a key role in reducing avoidable hospital utilization, supporting post-discharge care, and driving Medicare Advantage Stars and HEDIS performance. The RN will combine patient outreach and education with data-driven strategies to identify care gaps, engage patients and providers, and implement evidence-based interventions to improve overall health outcomes.
Clinical Care RN Responsibilities:
- Coordinate Transitions of Care, including post-discharge follow-up and readmission prevention
- Analyze clinical data (Athena EMR, DataHub) to identify gaps in care and support HEDIS/quality initiative
- Conduct patient outreach via phone/telehealth for education on chronic conditions, medications, and preventive care
- Perform medication reconciliation and assess patient understanding after discharge
- Collaborate with interdisciplinary teams to improve workflows and patient outcomes
- Support population health initiatives, especially for chronic conditions like diabetes and hypertension
- Document all patient interactions accurately in compliance with CMS and HIPAA standards
- Identify barriers to care and contribute to quality improvement efforts
- Bachelor’s degree in Nursing
- Active, unrestricted RN license (must be licensed in state of need)
- Minimum 3 years of clinical nursing experience (TOC, quality, managed care, or population health preferred)
- Experience with Medicare Advantage Stars, HEDIS, or value-based care preferred
- Proficiency with EMRs (Athena preferred) and Microsoft Office
- Strong communication, critical thinking, and patient engagement skills
- Ability to work independently in a remote setting
- Bilingual (English/Spanish or Creole) is a plus
- Must provide active RN license and full NURSYS report at time of submission
