**Care Manager - Access and Intake**
Seeking an organized and independent thinker who takes initiative to
meet client needs. This position is situated in Intake - the front door
of Howard Center, assisting clients at first contact and works with
clients from 6 months to a year. In this position you will be a part of
an enthusiastic team that will work to get services and provide support
to clients while providing the best customer service. Grow your
knowledge of services in the Chittenden County area and best practices
as you are trained to work across all client populations regardless of
age or diagnosis.

**Care Manager - Access and Intake**

Responsibilities
- Establish professional relationships with clients and families
- Provides case management to children, youth, and adults with a
