The Admission Nurse plays an important role in welcoming new residents to our community by completing nursing assessments, coordinating the admission process, and ensuring a smooth transition into care. This position works closely with residents, families, physicians, and the interdisciplinary team to support safe, personalized, and high-quality care from the beginning of each resident’s stay.

The Admission Nurse plays an important role in welcoming new residents to our community by completing nursing assessments, coordinating the admission process, and ensuring a smooth transition into care. This position works closely with residents, families, physicians, and the interdisciplinary team to support safe, personalized, and high-quality care from the beginning of each resident’s stay.
Schedule
- Full-Time: 11:00 AM – 7:00 PM
- Complete nursing assessments and admission documentation for new residents.
- Review medical records, physician orders, and resident care needs.
- Coordinate admissions with nursing staff, providers, residents, families, and referral partners.
- Communicate resident needs and updates with the interdisciplinary care team.
- Ensure accurate and timely completion of required clinical documentation.
- Assist with developing individualized care plans based on resident needs.
- Provide education, support, and reassurance to residents and families during the transition process.
- Promote a positive, welcoming experience for new residents and their loved ones.
- Current RN or LPN license in Pennsylvania required.
- Previous experience in long-term care, skilled nursing, admissions, or care coordination preferred.
- Strong clinical assessment, documentation, and organizational skills.
- Excellent communication and interpersonal skills.
- Compassionate, dependable, and committed to resident-centered care.
- Ability to work independently while collaborating with an interdisciplinary team.
