Position Overview
The Physical Therapist works under the direction of the Director of Rehabilitation and is responsible for evaluating, developing, and implementing individualized physical therapy programs for residents throughout the facility.
Key Responsibilities
- Provide physical therapy services to residents across the facility
- Conduct comprehensive physical therapy evaluations based on physician orders
- Develop and implement individualized treatment plans, including short- and long-term goals
- Deliver skilled therapy interventions and modify treatment plans as clinically indicated
- Participate in resident assessments and care plan development
- Collaborate with an interdisciplinary team to improve residents’ mobility, function, and overall quality of life
- Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctorate) recognized by APTA
- Current Physical Therapist license in good standing (required)
- Experience working with the geriatric population preferred
- Strong communication, documentation, and teamwork skills
