Intake Coordinator / Office Manager - Home Health

About the Role
Home Health Agency is seeking an experienced Intake Coordinator/Office Manager to manage referrals, insurance verification, and patient onboarding. This role ensures timely, compliant admissions and supports daily office operations. Must have strong home-health intake experience and advanced Data Soft Logic (DSL) skills.

Key Responsibilities
• Receive and process home-health referrals from hospitals, physicians, and community partners.
• Verify insurance benefits, obtain authorizations, manage re-auth cycles, and track visit utilization.
• Secure physician orders, track unsigned orders, and ensure timely SOC coordination with clinical staff.
• Enter and maintain accurate records in Data Soft Logic (DSL).
• Communicate with patients, families, clinicians, and referral sources regarding intake status.
• Support Administrator and DON with documentation, order management, and compliance tasks.
• Oversee office operations: phones, supplies, vendors, scheduling, and administrative workflow.
• Maintain HIPAA compliance and assist with weekend/on-call needs as required.

Requirements
• 2 years of home-health intake experience (required).
• 2 years of hands-on Home Health Electronic Medical Records (EMR) experience , preferably Data Soft Logic (DSL)(required).
• Strong knowledge of Medicare/Medi-Cal, commercial payers, authorizations, and home-health workflow.
• Excellent communication, multitasking, and organizational skills.
• Proficient in Microsoft Office, eFax, and general office systems.

Schedule
• Monday–Friday, 10:00 AM–4:00 PM
• Weekend/on-call as needed
