HA Home Care is a Medicare-certified, ACHC-accredited home health agency serving patients throughout North Hollywood, Los Angeles, and the San Fernando Valley. We're looking for an organized, detail-driven Intake Coordinator / Office Manager to be the front line of our patient onboarding process and the backbone of our office operations.
Pay: $20–$26/hour, depending on experience.
Schedule: Monday–Friday, 10:00 AM–4:00 PM (30 hours/week), with occasional weekend on-call for urgent referrals.

What you'll do:
- Review and triage new patient referrals from hospitals, physicians, SNFs, and families — assessing urgency and readiness for Start of Care
- Verify insurance, prior authorizations, and eligibility for Medicare Advantage, Medi-Cal, and commercial plans
- Track and secure physician orders, Plans of Care (POC/485), and recertifications
- Assign new patients to case managers and coordinate SOC timing with clinical supervisors
- Serve as the main point of contact for new patients and families during intake, and coordinate scheduling with field clinicians
- Keep our office running smoothly — supplies, vendors, filing systems, and day-to-day administrative workflow
- Maintain accurate, same-day documentation in Data Soft Logic (DSL)
What you bring:
- Experience with Data Soft Logic (DSL) or similar home health/EMR systems strongly preferred
- Working knowledge of home health regulations (Medicare CoPs, Medi-Cal) and payer requirements
- Strong triage judgment and comfort escalating clinical concerns appropriately
- Excellent organizational skills and calm, solution-oriented communication under pressure
- Proficiency in Microsoft Office and general office/IT troubleshooting
- Bilingual a plus
