Overview

We are seeking an experienced Home Health Licensed Practical Nurse (LPN) Nursing Supervisor to lead and oversee clinical operations within our home health agency. The ideal candidate will possess extensive nursing expertise, leadership skills, and a passion for delivering high-quality patient care. This role involves supervising nursing staff, ensuring compliance with regulations, and maintaining excellent patient outcomes across diverse care settings.

Duties

• Supervise, train, and evaluate nurses and caregivers in the field.
• Communicate with physicians, families, staff, and the interdisciplinary care team
• Review clinical documentation for accuracy, completeness, and compliance
• Assist with office-based clinical operations and case management responsibilities
• Oversee patient assessments, ensuring accurate documentation and adherence to clinical protocols.
• Ensure compliance with all federal, state, and local regulations governing home health care practices.
• Collaborate with interdisciplinary teams to develop personalized care plans that incorporate medical terminology, medication administration, IV infusion, tube feeding, and other specialized interventions.
• Lead quality assurance initiatives by monitoring patient outcomes and implementing continuous improvement strategies.
• Promote a positive work environment that encourages professional development and exceptional patient-centered care.
• Cover cases as needed.

Qualifications

• Active and unrestricted LPN license in the state of Virginia
• Home health or private-duty nursing experience
• Nursing supervisory or leadership experience preferred
• Excellent clinical judgment, communication, and organizational skills
• Reliable transportation and willingness to travel to clients’ homes
• Ability to work effectively in both field and office settings
• Dependable, detail-oriented, and committed to high-quality patient care

Pay: $35.00 - $37.00 per hour
