Duties
- Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe changes in health status, reporting findings accurately to healthcare professionals
- Support medication administration following care plans and HIPAA confidentiality standards
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Operate equipment such as Hoyer lifts and other mobility aids to ensure safe transfers and prevent injury
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces
- Document patient observations, care provided, and any incidents in compliance with healthcare regulations
Skills
- Proven experience in home care, assisted living, nursing homes, or group homes; familiarity with long-term care environments preferred
- Knowledge of patient observation techniques, vital signs measurement, and medication administration
- Strong understanding of dementia care, Alzheimer’s care, memory care, autism support, and working with individuals with developmental disabilities or disabilities in general
- Certified in CPR and First Aid; experience with hospice or palliative care is a plus
- Ability to perform heavy lifting safely using equipment like Hoyer lifts; physical stamina for extended periods of standing or walking
- Excellent communication skills to collaborate effectively with clients, families, and healthcare teams
- Demonstrated ability to follow detailed care plans while maintaining HIPAA compliance and respecting patient confidentiality
