Case Manager

Job Overview
We are seeking a dedicated and compassionate Case Manager to join our dynamic team. In this vital role, you will serve as a guiding force for our youth. The case manager is responsible for coordinating, monitoring, and documenting services for children and adolescents residing in group homes. The role ensures that each youth's individual service plan, treatment goals, educational needs, and behavioral supports are implemented in accordance with state regulations, licensing standards, and agency policies. The case manager serves as the primary liaison between the group home, placing agencies, families , schools and service providers.

Responsibilities

• Develop, implement, and monitor individual service plans (ISPs), treatment plans, and behavior support plans
• Ensure services meet the individual needs of each youth
• Coordinate and connect clients with community resources such as Section 8 housing, Medicare services, childcare programs, and other support systems.
• Coordinate care with therapists, schools, medical providers, and community resources.
• Monitor progress toward goals and update plan as needed.
• Maintain accurate , detailed and timely documentation of client interactions, progress notes, and service plans in compliance with organizational policies.
• Complete required documentation and training with staff on plans clans created included but not limited to progress:
• Progress Notes
• Monthly Summaries
• Incident follow-ups
• Service Plan updates
• Advocate for the needs, safety and well being of youth.
• Partcipate in traeatment team meetings, case meetings and court-related meetings as required.
• Montior youth behavio, adjustment and progress within the group home.
• Collabarate with direct care staff to ensure consisitent implementation of care plans.
• Maintain a regular monthly update to the following:
• Guardians
• Casweorkes
• Treatment Provider
• Cooridinate school enrollment, attandance and eduacational services.
• Support youth partciapation in the community actavities and extra curricualar programs.
• Ensure transportation, appointments and supports are scheduled and documented. Assist with appointments as needed.

Experience

• Bachelor's degree in Social Work, Psychology, Human Services, Criminal Justice, Education or related field.
• Familiarity with child welfare systems, including Childcare programs and Child Protective Services protocols.
• Minimum 1-3 years of experience working with youth in residential or foster care or behavioral settings.
• Familiarity with trauma informed - care and behavior management

Pay: $20.00 - $23.00 per hour
