Job Summary

Duties
- Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor residents’ vital signs and observe for any changes in health or behavior, documenting findings accurately in compliance with HIPAA regulations
- Administer medications following prescribed care plans and ensure proper medication storage and documentation
- Prepare nutritious meals tailored to individual dietary needs, ensuring proper food handling and cleanliness standards are maintained
- Provide companionship and emotional support to promote mental well-being and social engagement
- Maintain a clean and safe environment by performing regular cleaning duties within residents’ living spaces and common areas
- Support residents during community outings or activities to foster social integration and independence
- Follow all safety protocols including CPR and First Aid procedures; respond promptly to emergencies or incidents
Experience
- Prior experience working in assisted living, nursing homes, or group homes is highly preferred
- Experience working with individuals with developmental disabilities, autism, dementia, or Alzheimer’s is advantageous
- Knowledge of care plans, medication administration, patient monitoring, and vital signs measurement is essential
- Familiarity with HIPAA regulations to ensure confidentiality of patient information
- Hands-on experience with patient observation, dementia care, memory care, hospice care or home health services is beneficial
- Ability to perform heavy lifting safely when assisting residents with mobility or transfers using equipment like Hoyer lifts
- Strong communication skills for collaborating with team members, residents, families, and healthcare professionals
