Position: Hospice Response RN/LPN | Faith Hospice

Schedule

• Full-time | Mon-Fri evenings (2pm - 10pm)
• Part-time | every other weekend (8am - 8pm or 8pm - 8am)
  
  

Territory: Kent and Ottawa Counties and the outskirts of surrounding counties

Faith Hospice offers a unique opportunity for an experienced LPN to join our dedicated interdisciplinary team. As a Hospice Response Nurse, you will play a crucial role in providing compassionate care to patients at the end of life.

Responsibilities

• Respond to crisis calls and patient/family concerns
• Provide end-of-life visits
• Provide after-hours support and care
• Schedule:
• Full-time | Mon-Fri evenings (2pm - 10pm)
• Part-time | every other weekend (8am - 8pm or 8pm - 8am)
  
  

Location:

• Grand Rapids, covering a 55-mile radius around downtown Grand Rapids; Kent and Ottawa Counties and outskirts of surrounding Counties
  
  

Qualifications

• LPN licensure in Michigan
• Minimum 2 years of med/surg or acute care nursing experience
• Hospice or hospital experience preferred
• Passion for hospice nursing
• Valid Michigan driver's license, auto insurance, and reliable means of transportation for home visits
