At Ho‘ōla Nā Pua, our Bromley Family Pearl Haven Campus is not a shelter — it is a specialized residential treatment center serving girls ages 11–18 who have experienced complex trauma, including sexual exploitation.
We are seeking a Case Manager who understands that behind every file is a child with a story and that healing requires coordination, advocacy, and consistency across every system involved in the child's life.
What You’ll Do
As a Case Manager, you will serve as the central point of coordination for each youth’s care — ensuring that clinical, programmatic, and external systems are aligned to support her healing and long-term success.
This includes:
· Developing and managing individualized treatment and transition plans
· Serving as the primary liaison between youth, families, placing agencies, and external partners
· Coordinating and facilitating team meetings, case reviews, and court-related communication
· Maintaining accurate, timely, and compliant documentation
· Tracking progress, identifying gaps, and ensuring accountability across systems
· Participating in daily programming and maintaining a visible, relational presence with youth
· Upholding safety, ethical standards, and trauma-informed practices in all aspects of care
Who This Role Is For

Qualifications
· Bachelor’s degree (BA/BS) in Social Work, Psychology, Human Services, or related field required
· Must be at least 21 years of age
· Experience working with youth, behavioral health, or trauma preferred
· Ability to pass background check, drug screen, physical, and TB test
· Strong documentation, communication, and organizational skills
Compensation & Benefits
· Starting pay: $24/hour
· Medical insurance eligible the 1st of the month following hire
· Full benefits package available after 90 days
· 401(k) with up to 6% match after 1 year
