- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories using EMR (Electronic Medical Record) systems such as Epic or Cerner.
- Administer medications and injections accurately, including IV infusions and botulinum toxin treatments, following strict protocols and HIPAA regulations.
- Assist with complex procedures like catheterization, tube feeding, venipuncture, and sterile dressing changes while maintaining aseptic techniques.
- Monitor patients’ physiological responses during acute care situations such as ICU or emergency room scenarios, ensuring prompt intervention when necessary.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with diverse needs—including dementia care, Alzheimer's care, hospice, palliative care, and behavioral health cases.
- Provide patient education on health coaching topics like medication management, nutrition, discharge planning, and chronic disease management.
- Document all patient interactions thoroughly in medical records, ensuring compliance with ICD-10/ICD-9 coding standards and accurate medical documentation practices. Skills
- Proven experience working in critical care environments such as ICU or Level I trauma centers; familiarity with Level II trauma center standards is a plus.
- Extensive knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques.
- Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks for efficient documentation and patient management.
- Skilled in performing triage assessments in urgent care or outpatient settings to prioritize patient needs effectively.
- Experience with specialty areas such as geriatrics, pediatrics—including neonatal ventilator management—and home & community care environments.
- Strong understanding of infection control protocols and sterile processing procedures to ensure patient safety.
- Ability to work with individuals with disabilities or developmental delays across various settings like assisted living or long-term care facilities.
- Excellent communication skills for collaborating with healthcare teams and educating patients on their treatment plans.
