What You’ll Do
Role
As a Social Services Specialist, you’ll play a key role in ensuring safe, thoughtful, and well-coordinated transitions of care by:
- Meeting with patients and families at admission to complete paperwork and identify psychosocial needs
- Coordinating care transitions from admission through discharge in collaboration with the interdisciplinary team
- Completing referrals, assessments, care plans, progress notes, and required discharge documentation
- Communicating care recommendations and transition plans clearly with patients and families
- Providing counseling, education, and advocacy to protect patient rights and support decision-making
- Assisting with insurance-related matters including Medicare/Medicaid notifications, denials, and copays
- Responding to patient and family concerns promptly and professionally
- Educating care teams on identifying social service needs and available resources
- Participating in committees and meetings as assigned
- Bachelor’s degree in a healthcare-related field preferred
- Minimum of 2 years’ experience in social work, case management, or a related field
- Experience with discharge planning or care transitions in post-acute or acute care settings preferred
- Strong communication, organization, and documentation skills
- Ability to remain calm, flexible, and compassionate in emotionally charged situations
- Comfort using technology in a fast-paced healthcare environment
- Ability to lift up to 35 pounds
- Ability to sit, stand, bend, and move throughout the day
- Must be able to read, write, and speak English
