Help us help others:

A Mental Health Care Coordinator will assess needs, develop treatment plans, and effectively collaborate with individuals and their family members. The Care Coordinator will assess strengths and needs and, in collaboration with the individual, develop a person-centered recovery plan.  The Care Coordinator will provide skills training, crisis prevention/management, linkage, and referral to support goal progress for children and families living with mental health challenges.  This compassionate, caring, and supportive team member will contribute to the skills improvement, achievements, and overall potential success of a child and their state of well-being.   

You will get to:

• Assess individual needs and collaborate with them to develop and execute effective recovery and life management plans.
• Provide relevant skills training to improve daily life.
• Collaborate with service providers and agencies to be a liaison for effective care and positive outcomes.
• Write and document articulately while using correct grammar so that documentation is clear and valuable.
• Maintain compliance with all legal requirements and company policies and procedures.
• Consistently meet with individuals to maintain progress as well as compliance.

You'll definitely need:

• At least one year of experience implementing the Child and Adolescent Needs and Strengths Assessment or the Texas Resilience and Recovery Model for Adults or Children.
• A bachelor's degree from an accredited college or university in psychology, sociology, behavioral or human services, or a related degree, with the ability to credential as a Qualified Mental Health Professional 
• A valid Texas State Driver's License.
• Current automobile insurance and the ability to travel to individuals when necessary.

We'll also want you to:

• Be available to manage crisis situations as they occur in sometimes off-scheduled hours (this is typically an infrequent occurrence)
• Employ excellent verbal and written communication skills to engage and report effectively.
• Maintain a well-organized schedule while allowing room for flexibility and adjustments.
• Maintain accurate and timely collaborative documentation.
• Sustain a calm demeanor when under pressure to meet deadlines and handle sometimes difficult situations.
• Provide exemplary customer service and care to individuals with their best interests at heart.
• Work autonomously to complete assignments within company and state parameters with a high level of quality. 
• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
