RN Case Manager – Home Health (GA)
Schedule: Monday–Friday, 8:30 AM–5:00 PM
License: Active, unencumbered GA RN License required
Productivity: 30 units per 40-hour week
Mileage Reimbursement: $0.43/mile
Coverage Area: Chatham, Liberty, Bryan, and Effingham Counties
Position Overview:
The RN Case Manager provides comprehensive, patient-centered care to high-risk members in a home health setting. This role focuses on clinical assessment, care coordination, and driving effective, cost-efficient patient outcomes.
Key Responsibilities:
- Perform comprehensive patient assessments and develop individualized care plans
- Collaborate with primary care providers and interdisciplinary teams to coordinate care
- Continuously evaluate and adjust care plans to ensure appropriate and cost-effective treatment
- Coordinate care transitions, including hospital discharges and ongoing services
- Arrange non-medical support (e.g., transportation, housing) to support treatment compliance
- Communicate with physicians, social workers, discharge planners, and care teams
- Engage specialty resources to optimize patient outcomes
- Maintain accurate documentation of clinical, functional, and financial outcomes
- Promote health education, disease prevention, and wellness initiatives
- Identify and prevent adverse patient events; intervene as needed
- Advocate for patients while maintaining confidentiality and compliance standards
Qualifications:
- Associate’s or Bachelor’s Degree in Nursing
- Active, unrestricted RN license in Georgia
- Case Management certification preferred
- Strong clinical judgment and ability to analyze complex medical information
- Proficiency with OASIS documentation and HomeCare HomeBase (HCHB) preferred
