Registered Nurse (RN) – Home Health
Schedule:
Monday – Friday, 8:00 AM – 5:00 PM
Weekend and on-call rotation required
On-call expectation includes at least 1 weekend per month
Coverage Area:
Harrison, Marion, Taylor, Upshur, Lewis, Doddridge, and Barbour Counties
Productivity Expectation:
30 points per week
Mileage Reimbursement:
$0.54 per mile
Position Overview
We are seeking an experienced Home Health Registered Nurse to provide skilled nursing care, case management, and health education to patients in a home-based setting. This role focuses on assessing patient needs, implementing care plans, coordinating with interdisciplinary teams, and promoting health maintenance and disease prevention.
Key Responsibilities
- Conduct patient assessments and implement individualized nursing care plans
- Provide case management and coordinate care with physicians, discharge planners, and healthcare professionals
- Observe, assess, plan, implement, and evaluate nursing care for chronic, acute, and medically complex patients
- Monitor patient conditions and communicate status changes to physicians and care teams
- Ensure proper medication management, including monitoring, documentation, storage, administration oversight, and disposal procedures
- Maintain accurate and timely nursing documentation, including daily, monthly, quarterly, and annual reports as required
- Coordinate healthcare appointments and ensure follow-up care is completed appropriately
- Maintain communication with inpatient facilities and discharge coordinators during hospitalizations
- Supervise and support LPN/LVN staff as needed
- Assist with training and education for new employees and unlicensed staff regarding health, safety, and disease prevention protocols
- Ensure infection prevention standards are followed regarding equipment, supplies, and patient care environments
Required Qualifications
- Active West Virginia RN license
- Previous home health experience required
- Experience with HCHB (Homecare Homebase) required
- OASIS documentation experience required
- Strong clinical assessment and case management skills
- Ability to work independently and collaboratively within interdisciplinary teams
Preferred Qualifications
- Experience managing medically complex and high-risk patients
- Strong communication, organizational, and documentation skills
- Supervisory or staff training experience preferred
