Registered Nurse Case Manager — URGENT NEED!
Schedule: Monday–Friday | 8:00 AM–5:00 PM
Coverage Area: Orange, Durham & Alamance Counties, NC
Benchmark: 6 patients/day
Mileage: $0.41/mile
License: Active RN license; Compact licenses accepted
We are seeking an experienced Registered Nurse Case Manager to provide comprehensive, patient-centered care to high-risk members across the community.
Key Responsibilities
- Assess high-risk patients and develop individualized care plans.
- Coordinate care with physicians, social workers, discharge planners, and other healthcare professionals.
- Manage care transitions and connect patients with medical and community resources.
- Monitor clinical, functional, and financial outcomes.
- Promote health, prevent complications, and advocate for patients.
- Maintain accurate documentation while protecting patient privacy and confidentiality.
Requirements
- Associate’s or Bachelor’s degree in Nursing or related field.
- Current, unrestricted RN license; Compact license accepted.
- Case Management certification.
- Strong clinical assessment and critical-thinking skills.
- Med Surg or Critical Care experience preferred.
- Ability to manage complex medical information and make sound clinical decisions.
