Registered Nurse Case Manager – Home Health
Schedule:
Monday – Friday, 8:00 AM – 5:00 PM
No on-call requirements
40 hours guaranteed weekly
Coverage Area:
Orange, Durham, and Alamance counties
Compensation Details:
Mileage reimbursement provided at $0.48/mile
Patient Volume Expectation:
Average of 6 patients per day
Position Overview
We are seeking an experienced Registered Nurse Case Manager to support high-risk members in a home health setting. This role is responsible for conducting comprehensive assessments, coordinating care plans, and collaborating with interdisciplinary teams to ensure optimal patient outcomes and continuity of care.
Key Responsibilities
- Perform comprehensive assessments for high-risk patients in the home health setting
- Develop and coordinate individualized care plans in collaboration with primary care providers and interdisciplinary teams
- Monitor and reassess patient needs to ensure appropriate and cost-effective levels of care
- Coordinate care transitions and collaborate with physicians, social workers, discharge planners, and other healthcare professionals
- Arrange non-medical support services such as transportation and housing assistance when needed
- Identify opportunities for health promotion, disease prevention, and improved patient outcomes
- Maintain detailed documentation of clinical, functional, and financial outcomes
- Advocate for patients while maintaining privacy and confidentiality standards
- Intervene quickly to minimize adverse patient outcomes when necessary
Required Qualifications
- Active RN license (compact license accepted)
- Associate’s or Bachelor’s degree in Nursing or related field
- Previous home health experience required
- Experience with HCHB and OASIS documentation required
- Strong assessment and case management skills
- Ability to analyze complex medical information and make sound clinical decisions
Preferred Qualifications
- Med-Surg and/or Critical Care experience preferred
- Case Management certification preferred
Salary : $54 - $57
