Pharmacist – Home Infusion Pharmacy
Schedule: Monday–Friday, 9:00 AM – 5:30 PM
Flexibility Required: Occasional extended hours up to 7:00 PM based on patient and business needs.
Position Overview
We are seeking an experienced Home Infusion Pharmacist to join our growing pharmacy team. This role is responsible for ensuring the safe and accurate dispensing of medications, performing final prescription verification, reviewing laboratory results, maintaining documentation, and supporting patient care initiatives. Candidates must have prior home infusion experience, including strong knowledge of sterile compounding and clean room operations.

Required Qualifications
- Doctor of Pharmacy (PharmD) degree
- Active Pharmacist license in good standing
- Home infusion pharmacy experience required
- Sterile compounding and clean room experience required
- Strong knowledge of pharmacy regulations, medication management, and patient counseling
- Excellent communication and documentation skills
Responsibilities
- Perform final verification of compounded medications and prescriptions
- Review laboratory results and clinical documentation as needed
- Ensure compliance with all state and federal pharmacy regulations
- Maintain accurate and confidential patient records
- Counsel patients on medication usage, storage, dosage, side effects, and potential drug interactions
- Identify and resolve medication-related issues in collaboration with physicians and healthcare providers
- Prepare and oversee sterile compounding activities in accordance with established procedures
- Monitor medication therapy and patient compliance
- Maintain appropriate pharmaceutical inventory and supplies
- Educate patients on the proper use of medical devices and treatment therapies
- Supervise pharmacy technicians and support staff
- Collaborate with physicians, nurses, pharmacists, and other healthcare professionals to optimize patient outcomes

- Experience in home infusion, specialty pharmacy, or hospital pharmacy settings
- Strong attention to detail and commitment to patient safety
- Knowledge of USP sterile compounding standards
- Ability to work independently and manage multiple priorities in a fast-paced environment
