Schedule & Coverage
- Monday–Friday, 8:00 AM – 4:30 PM
- Weekly case conference meeting every Wednesday at 8:45 AM
- Productivity benchmark: 30 points per week
- Mileage reimbursement provided at $0.43 per mile
- Coverage area includes Hudson County, New Jersey
Position Overview
We are seeking a compassionate and dependable Certified Home Health Aide (CHHA) to provide high-quality personal care and daily living assistance to patients in their homes. The ideal candidate is patient-focused, organized, and committed to supporting individuals with dignity and respect while promoting comfort, safety, and independence.
Key Responsibilities
- Assist elderly, disabled, convalescent, and chronically ill patients with activities of daily living
- Provide personal care assistance including bathing, grooming, dressing, toileting, and mobility support
- Perform light housekeeping duties such as making beds, laundry, dishwashing, and maintaining a clean living environment
- Prepare meals and assist with feeding when necessary
- Observe and report changes in patient condition or behavior to the appropriate clinical staff
- Provide companionship and emotional support to patients and families
- Educate patients and families on nutrition, cleanliness, and household safety practices as appropriate
- Participate in weekly case conference meetings and maintain effective communication with the care team
- Maintain accurate documentation and comply with all company policies and patient confidentiality standards
Required Qualifications
- Active Certified Home Health Aide (CHHA) certification in the state of New Jersey required
- Current CPR certification required
- Ability to work independently and manage time effectively
- Strong interpersonal and communication skills
- Reliable transportation and willingness to travel throughout assigned coverage areas
Preferred Skills & Experience
- Previous home health experience preferred
- Experience with HCHB (Homecare Homebase) preferred
- Familiarity with Microsoft Teams and Outlook preferred
- Compassionate, dependable, and patient-centered approach to care
Salary : $21 - $23
