Home Health Registered Nurse (RN) – Case Management
Schedule:
Monday – Friday, 8:00 AM – 5:00 PM
Some weekend availability preferred (approximately 1 weekend per month and 1 on-call night per week)
Coverage Area:
Escambia, Santa Rosa, Okaloosa, and Walton Counties
Primary service area will focus on Okaloosa and Walton Counties
Mileage Reimbursement:
$0.43 per mile
Position Overview
We are seeking a skilled and compassionate Home Health Registered Nurse to provide high-quality patient care and case management services in the home setting. This role involves conducting comprehensive patient assessments, coordinating individualized care plans, and collaborating with interdisciplinary teams to support positive patient outcomes.
Key Responsibilities
- Conduct comprehensive assessments for high-risk patients in the home health setting
- Develop and implement individualized care plans in collaboration with physicians and care teams
- Coordinate patient care transitions and communicate with providers, social workers, discharge planners, and other healthcare professionals
- Identify and coordinate supportive services to promote treatment compliance and patient stability
- Monitor patient progress and reassess care needs to ensure appropriate and cost-effective treatment
- Maintain accurate and timely clinical documentation and OASIS assessments
- Support health promotion and illness prevention initiatives
- Advocate for patient privacy, confidentiality, and quality of care
- Respond proactively to changes in patient condition to minimize adverse outcomes
Required Qualifications
- Active Florida RN license or compact RN license
- Associate’s or Bachelor’s Degree in Nursing or related field
- Home health experience required
- Strong experience with OASIS documentation and HCHB/Home Care Home Base required
- Ability to assess complex clinical situations and make sound nursing decisions
- Competency in Oasis collection and PointCare/Home Care Home Base systems
