Registered Nurse (RN) — Home Health Case Manager Kenosha, Racine & Southeast Milwaukee County, WI | M–F, 8:00am–5:00pm | No On-Call | 40 Hours Guaranteed

Schedule & Logistics

• Monday–Friday, 8:00am–5:00pm — no on-call
• 40 hours per week guaranteed
• 5 visits per day
• Mileage reimbursement: $0.53 per mile
• Counties covered: Kenosha, Racine & Southeast Milwaukee County

Required Qualifications

• ✅ Active Wisconsin RN License (required)
• ✅ Hospice experience (required)
• ✅ Wound care experience (required)
• ✅ HCHB experience highly desired; Matrix Care or any EMR acceptable
• ✅ CPR/BLS certification (current)
• ✅ Valid driver's license, reliable vehicle & current auto insurance
• ✅ Associate's or Bachelor's Degree in Nursing or related field
• Case Management certification preferred

Compliance Requirements

The following skills competency documents are required and must be uploaded during compliance checks:

• Elder Abuse Training
• HIPAA Training
• Infection Prevention & Control
• OSHA Compliance
• Education/skills training received within the past 12 months (include copies of any training certificates)

What You'll Do

• Perform comprehensive assessments of high-risk members to identify clinical, functional, and social needs
• Collaborate with primary care providers to develop and implement individualized, comprehensive care plans with targeted interventions
• Continuously reassess and address appropriate, cost-efficient levels of care as patient needs evolve
• Coordinate non-medical support services such as housing and transportation to ensure compliance with treatment plans
• Manage care transitions and coordinate with physicians, social workers, discharge planners, and claims professionals to move patients to the appropriate level of care
• Engage specialty resources as needed to achieve optimal patient outcomes
• Maintain detailed records of clinical, functional, and fiscal outcomes throughout the care management process
• Identify opportunities for health promotion and illness prevention
• Prevent adverse patient occurrences when possible and intervene quickly to minimize poor outcomes
• Serve as a patient advocate, upholding privacy, dignity, and confidentiality at all times
