Home Health LPN
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
- Evening & weekend on-call rotation required
License Requirement:
- Active West Virginia LPN License
Productivity:
- 32 points per week
Mileage Reimbursement:
- Yes
Coverage Area:
- Harrison, Marion, Tyler, Barbour, Doddridge, Lewis, Upshur Counties
Preferred Experience:
- Prior home health experience
Key Responsibilities
- Provide patient care in accordance with the established plan of care while promoting independence and dignity
- Monitor vital signs and report any abnormal findings
- Administer medications, treatments, feedings, oxygen, and ostomy care as required
- Collaborate with interdisciplinary team members including PT, OT, and Speech Therapy
- Maintain strict infection control practices and report any concerns
- Accurately document all patient care, communication, and progress toward goals
- Ensure timely ordering of supplies, medications, and equipment
- Follow physician orders and ensure all patient needs are met
- Monitor and document incidents, completing reports and notifying appropriate personnel
- Assess patients following incidents and ensure proper follow-up
- Maintain medical equipment and coordinate repairs with DME providers
Qualifications
- Active WV LPN license
- Strong clinical and documentation skills
- Ability to work independently in a home setting
- Excellent communication and patient care skills
Salary : $30 - $38
