Home Health Licensed Practical Nurse (LPN)
Schedule: Monday–Friday, 8:00 AM–5:00 PM (40 hours/week)
On-Call: 1 day per week
Mileage Reimbursement: $0.49 per mile
Territory:
• Escambia & Santa Rosa Counties OR
• Okaloosa & Walton Counties
Job Summary
We are seeking a compassionate and dependable Home Health LPN to provide high-quality, patient-centered care in the home setting. This role focuses on supporting patients’ health, independence, and dignity while working collaboratively with an interdisciplinary care team.
Responsibilities
- Provide skilled nursing care according to the patient’s plan of care.
- Monitor vital signs and report abnormal findings to the care team.
- Administer medications, oxygen, feedings, and provide ostomy and wound care as ordered.
- Carry out care routines established by Physical, Occupational, and Speech Therapists.
- Maintain and enforce infection control standards and report potential communicable diseases.
- Document all patient care, communication, progress, and outcomes accurately and timely (HCHB/OASIS).
- Order and manage supplies, medications, and feedings to ensure adequate availability.
- Ensure patient needs are met in accordance with physician instructions.
- Monitor and document incidents; complete incident reports and notify appropriate personnel.
- Inspect and maintain medical equipment; report faulty equipment for repair.
Required Qualifications
- Active LPN License (required)
- Home Health nursing experience preferred
- Experience with wound care, patient assessment, and medication administration
- Experience with HCHB and OASIS documentation preferred
- Strong clinical judgment, communication, and time-management skills
- Valid driver’s license and reliable transportation

- Amount of experience: Open / flexible — willing to discuss based on overall skill set and home health experience
