Responsibilities
- Provide direct patient care to long-term care residents under supervision of an RN and physician per care plans
- Administer prescribed medications and document administration in the electronic health record
- Monitor patient status, vital signs, and respond to changes with appropriate escalation
- Assist with activities of daily living, mobility, wound care, and routine procedures as needed
- Collaborate with the interdisciplinary team to ensure patient safety, comfort, and quality of care
- Document accurately and maintain infection control standards and patient safety practices

Requirements
- Hawaii Licensed Practical Nurse license active by start date (if not currently licensed, must apply upon offer acceptance)
- Prior LPN experience in a long-term care setting
- Ability to work 12-hour shifts on a rotating schedule
- Must be travel-ready and able to travel to rural Hawaii for the assignment
- 13-week assignment with flexibility to accommodate on-site needs
- Must arrive the Thursday prior to start date for employee health clearance
