Responsibilities

• Perform nursing assessments and contribute to care planning for residents in a long-term care setting
• Administer medications and treatments as prescribed; monitor patient status and notify MD/NP of pertinent changes
• Complete admissions and discharges; assist residents with activities of daily living
• Provide gastrostomy tube care, urinary catheter care, nebulized therapy, wound vac care, and CPAP care as assigned
• Maintain clear documentation; coordinate with the physician, NP, and interdisciplinary team
• Uphold safety and infection control standards; respond to resident needs and changes in condition promptly
  
  

Requirements

• Active LPN license (state as applicable; current license required)
• Experience in a long-term care setting
• Ability to work in a rotating/variable shift schedule based on census
• Flu vaccination proof or facility-approved medical/religious exception prior to contract start
• Availability to arrive the Thursday before start for employee health clearance
