Responsibilities

• Conduct comprehensive SLP evaluations and screen residents for communication and swallowing concerns
• Develop, implement, and monitor individualized treatment plans addressing speech, language, cognitive-communication, and swallowing impairments
• Provide direct therapy to residents (1:1 and small groups) and adjust interventions based on progress and goals
• Document evaluation results, progress notes, and discharge plans in the electronic health record; communicate with the interdisciplinary team
• Educate residents, families, and caregivers about goals, progress, and strategies to support communication and safe swallowing
  
  

Requirements

• Active Speech-Language Pathology license for California or eligibility to obtain licensure
• Ability to work in a skilled nursing facility setting and float to assigned locations as needed
• Strong clinical skills in assessment, treatment planning, and documentation for SNF residents
• Excellent communication and collaboration with an interdisciplinary team
• Ability to manage multiple cases and maintain accurate, timely documentation
