Responsibilities

• Administer medications and treatments per the plan of care and document responses in the electronic record
• Implement rehabilitation protocols and therapies in collaboration with physical, occupational, and speech therapy to promote patient mobility and independence
• Monitor patients’ conditions, perform regular assessments, and communicate changes to the interdisciplinary team
• Ensure safety, infection control, wound care where applicable, and fall risk mitigation on all shifts
• Provide education to patients and families about mobilization strategies, activity programs, and progress toward goals
• Document thoroughly and maintain clear, concise communication within the care team to support coordinated, high-quality care
  
  

Requirements

• Current RN license (state-specific) and eligibility to practice in New Hampshire or as required for the assignment
• Relevant experience in rehabilitation or skilled nursing facility settings preferred
• Ability to work 3x12-hour night shifts and adhere to the stated schedule
• Strong assessment, observation, documentation, and communication skills
• Demonstrated ability to collaborate with an interdisciplinary team and support care plan updates
