Shift: Day shift, 8 hours per day, 5 days per week (Mon–Fri, 7:00 am–4:00 pm)

Responsibilities

• Take patient histories and document in the electronic health record
• Measure vital signs and assess patient status; report changes to the supervising clinician
• Administer prescribed medications and injections; monitor for adverse reactions
• Assist clinicians during examinations and minor procedures
• provide patient education on med administration, treatments, and follow-up instructions
• Triage phone and in-person patient concerns; coordinate with the care team to ensure timely care
• Maintain infection control protocols and stock medical supplies; support practice workflow and documentation
  
  

Requirements

• Active New York LPN license
• BLS certification
• Minimum 1 year of LPN experience
• Graduation from an accredited practical nursing program (LPN diploma)
• Ability to work the day shift in a primary care office setting (Mon–Fri, 7:00 am–4:00 pm)
• Strong communication and teamwork skills; ability to work in a fast-paced office environment
