Responsibilities

• Provide direct patient care to residents in a long-term care setting, including ongoing assessments and monitoring
• Administer medications and treatments per physician orders with accurate documentation
• Collaborate with an interdisciplinary team to develop, implement, and adjust individualized care plans
• Monitor safety, comfort, and response to care; assist with activities of daily living as needed
• Document nursing assessments, care plans, progress notes, and changes in the resident’s condition in the medical record
• Respond promptly to changes in resident status and communicate significant updates to the care team and supervising clinicians
