This travel assignment involves a two-site schedule in the Tehachapi/Bakersfield area: Mon/Wed/Fri at one site and Tue/Thu at a second site within the same care network. Ideal for a clinical dietitian who thrives in interdisciplinary teams and patient-centered home-based hospice care.

Responsibilities

• Conduct nutrition assessments for hospice patients within the home care setting and document results
• Develop, implement, and monitor individualized medical nutrition therapy and nutrition care plans
• Collaborate with the interdisciplinary team to support symptom management, comfort, and quality of life
• Provide patient and family education on diet, feeding assistance, and nutrition-related goals
• Monitor and document patient progress, adjust care plans as needed, and communicate changes to the care team
• Ensure compliance with relevant professional standards and licensing requirements in all documentation
  
  

Requirements

• Licensure: Registered Dietitian (RD) in California; RD licensure required within 6 months of completion of residency program
• Education: Bachelor’s degree in food and nutrition or equivalent required; Master’s degree required only if eligibility for examination began on or after 01/01/2024
• Dietetics training: Dietetic internship or equivalent required
• Certifications: AHA-BLS or ARC-BLS required
• Experience: Clinical Dietitian experience preferred
• Additional: Must be able to work in a home healthcare – hospice setting
