Responsibilities

• Provide comprehensive nutritional care to patients identified with nutritional risk or special needs, including initial assessment and ongoing reassessment
• Develop, implement, and monitor individualized nutrition plans of care; document all interventions in accordance with policy and standards
• Educate patients and families on nutrition plans, dietary management, and relevant health considerations
• Communicate effectively with an interdisciplinary care team to coordinate nutritional care and optimize patient outcomes
• Demonstrate clinical competency and sound decision-making in delivering nutrition care within a home-based hospice setting
• Maintain accurate, timely documentation of assessments, interventions, and outcomes in the patient record
  
  

Requirements

• Active or eligible licensure as a Registered Dietitian (Clinical) in the state of California
• Clinical nutrition experience, with emphasis on assessment, care planning, and documentation
• Ability to work in a home healthcare - hospice setting and travel to patient locations as needed
• Strong communication skills for patient, family, and interdisciplinary team interactions
• Proficiency in developing and documenting nutrition care plans in line with policy and standards
